home support program
If you or a loved one have been diagnosed with a life-limiting illness and given less than six months to a year to live, we are here for you. We support you in living with dignity and peace. We understand the challenges of facing a life-threatening illness. Our goal is to enhance the quality of life, and reduce suffering for both you and your circle of care.
our approach
We connect each client with carefully screened, trained, and supervised volunteers who provide emotional support and companionship. Our services are led by a Nurse Coordinator and dedicated care team that advocate for your needs and help connect you with appropriate community services and professional care, in a timely and responsive manner.
If you’re unsure about eligibility for our services, our Nurse Coordinator is available to answer any questions and to connect you with the care that best meets your needs. You don’t have to navigate this alone; we’re here to support you.

Volunteer Companionship: Compassionate, skilled volunteers provide companionship, comfort measures, practical help, and emotional support. Each volunteer undergoes intensive and hands-on training and offers a compassionate, neutral, and non-judgmental presence.

Your journey begins with our Nurse Coordinator who will work closely with you to assess your needs and oversee the support provided by our volunteers.

Anyone can refer themselves, a family member, or a friend to our Home Support Program. Simply fill out the referral form at the top of the page (coming soon) or contact us for more information.
